Fast Track Physiotherapy - Terms & Conditions
1. You will be examined and treated in line with the Rules of Professional Conduct and Core Standards of Physiotherapy Practice as set out by the Chartered Society of Physiotherapy.
2. You will be examined by a physiotherapist registered with the Health Professions Council.
3. Following examination, the physiotherapist will explain their assessment of your condition and propose an appropriate treatment plan. This will be described to you, outlining any risks and where possible, an estimated number of treatment sessions.
4. All treatment plans are subject to change as your condition progresses. These changes will be discussed with you before any further action is taken.
5. You may decline any treatment at any time without prejudice.
6. Your appointment times will be scheduled according to your requirements and to the best of our ability. These may be subject to change due to staff illness and you will be advised at the earliest opportunity if this is the case.
7. Your personal details are treated in the strictest confidence. Medical records will be hand written and kept securely.
8. If you arrive late for your appointment, your treatment session will be shorter.
9. Failure to attend an appointment will incur a charge. (How much?)
10. If you wish to cancel your appointment we need 24 hours notice.
11. If we have not agreed the number of sessions to be provided, we shall provide sessions on a session-by-session basis. We will agree the date and time of the first session and any subsequent sessions by telephone, in person or by e-mail.
12. We shall seek to enable you to achieve your desired outcomes. No outcome can be guaranteed, however, and you have sole responsibility for acting on any recommendations or advice the physiotherapist may give. We have no liability for any loss incurred by you, whether financial or otherwise, following our provision of the Services, nor for any perceived failure by you, whether justified or otherwise, to achieve your desired outcomes or goals.
13. Our liability to compensate you for any loss or damage (in the case of loss or damage other than death or personal injury) is limited to a reasonable amount (and not exceeding the amount you pay in total for the Services), having regard to such factors as whether the damage was due to a negligent act or omission by Fast-Track Physiotherapy. Nothing in these conditions will limit our liability for death or personal injury caused by our negligence or the negligence or our employees, agents or subcontractors or fraud and fraudulent misrepresentation.
14. We shall not be liable to you, whether in contract, tort (including negligence), breach of statutory duty, or otherwise, or for any loss of profit, or for any indirect or consequential loss arising under or in connection with the Services. Except as set out in these conditions, all warranties, conditions and other terms implied by statute or common law are, to the fullest extent permitted by law, excluded from the contract. This clause will survive the termination of the contract.
15. Treatment offered is at the physiotherapist’s discretion and will be based on the most recent research and guidelines available.
16. Should clients decline the first appointment offered within 48 hours then we cannot guarantee that the next offered appointment will be within that 48 hour period.
17. Not all treatments will be included within the monthly fee and clients will be advised which treatments would incur an extra charge and can refuse these treatments.
